Tilak Road, Pune
Gripping, lifting and standing for hours can leave a tendon at the elbow or a band under the heel sore for months. Therapeutic ultrasound is a gentle treatment that physiotherapists sometimes add. A physiotherapist visits your home on Tilak Road, assesses the area, uses ultrasound if it suits, and focuses on exercise and load changes, which do most of the work.
Behind a counter or in a kitchen, some parts of the body work harder than anyone notices: the tendon at the outer elbow that grips and lifts, the thick band under the heel that takes every step on a hard floor. Overload them for weeks and they become sore, then stiff, then hard to ignore. Therapeutic ultrasound is one of the tools that physiotherapists use for stubborn problems of this kind.
The note below covers what the treatment is, what it can and cannot do, and how it might fit into a home visit on Tilak Road. For the overall service, see our ultrasound therapy page.
The word confuses people, because the ultrasound used in pregnancy is a scan. Therapeutic ultrasound is a treatment. A hand-held probe is moved over a smear of gel on the skin, and it sends high-frequency sound waves into the tissue beneath. The waves cause tiny vibrations that can produce a gentle warmth, or, in the pulsed mode, little or no heat. The idea is to encourage healing and ease pain in tendons, ligaments and muscle.
The honest summary of the evidence is that it is mixed: a small benefit is reported for certain conditions, and for others it does no better than a dummy treatment. Good guidelines treat exercise as the main treatment for tendon problems and see ultrasound, at most, as a minor extra. If your physiotherapist offers it, expect it to be a short part of a plan whose main work is graded loading and changes to how you use the area.
The tendon problems are mostly managed with exercise, which is why the sports injury physiotherapy and manual therapy pages are relevant companions.
Ultrasound is not used over a suspected fracture site that has not been assessed, over areas with active infection or a tumour, over an area where a blood clot is suspected, or over the abdomen of a pregnant woman. It is avoided over the eyes, the heart and areas of reduced sensation, where you might not notice the heat. Over metal implants, the physiotherapist uses caution, and anyone with a pacemaker is not treated over the chest. Children and teenagers with growing bone plates are treated differently. The screening comes first, and if there is any doubt, the treatment is skipped.
The physiotherapist examines the sore area, tests which movements provoke it and decides if ultrasound adds anything. If so, gel is applied, and the probe is moved slowly over the area for a few minutes. You should feel nothing, or a mild warmth; any burning or sharp pain is a reason to stop. Because the equipment is bulky, whether a unit comes with the physiotherapist is confirmed at booking, and the plan does not depend on it.
Most of the visit is spent on the part that matters: strengthening the tendon with slow, heavy-enough exercises, changing how you grip or stand, and planning rest breaks. For an elbow, that can mean changing how you lift cartons, using both hands, and keeping the wrist straight. For a heel, it means looking at the footwear and the floor you stand on, adding a cushioned insole if useful and stretching the calf and foot.
See a doctor if the pain follows a fall or a blow, if the area is hot, red and swollen, if there is fever, or if you notice numbness or weakness in the hand or foot. Sudden severe pain at the back of the ankle with a snapping feeling needs urgent assessment.
