Tilak Road, Pune
Checking mirrors in heavy traffic, wearing a helmet and bending over a phone or a ledger can leave the neck and upper back stiff and sore. Manual therapy uses skilled, gentle hands-on movement to ease that stiffness. A physiotherapist visits your home on Tilak Road, assesses first, treats if it is suitable and follows it with exercises that make the change last.
A stiff neck is a familiar companion of city riding. You turn your head to check the mirror, then the blind spot, then the mirror again, and by the time you reach home you cannot look over your shoulder without a twinge. The same thing can happen to a person who spends the day looking down at a phone, a ledger or a sewing machine.
Manual therapy is the hands-on part of physiotherapy: skilled, graded movement of joints and soft tissue to ease pain and restore motion. Our manual therapy page describes the approach in general. This page is about what it can offer for the stiff necks and tight upper backs of people who live and work along Tilak Road, and how it is used at home.
The physiotherapist uses the hands to apply controlled movement to a joint, usually in gentle, repeated oscillations, to find which direction is stiff and which is painful. In the neck and upper back, this is often a small gliding movement of one vertebra on the next, applied while you lie comfortably. Soft-tissue work on the muscles that hold the neck and shoulder blades can be added. The aim is to improve movement and reduce pain for a time, so that the exercises that follow are easier and more effective.
The evidence says that manual therapy gives short-term relief for many people with neck and back pain, and that it works better when combined with exercise and advice instead of being used alone. It does not "put bones back in place", and a click or pop is not a sign that something has been fixed. Quick, forceful thrust techniques are used rarely, only where they are suitable, and never without a screening.
When shoulder stiffness is the main problem, the frozen shoulder physiotherapy page covers how manual work fits in. For tender knots, dry needling is a different approach.
Manual therapy is well suited to a home visit. A firm bed or a sturdy sofa is enough, with a pillow or two for support. The physiotherapist will assess the neck first, testing turning, bending and the positions that bring on pain, and checking for arm symptoms such as numbness or weakness. If the findings are reassuring, a few minutes of hands-on treatment come first, followed by retesting to see what changed. Then come the exercises: chin tucks, shoulder blade control, rotation practice and stretches for the muscles at the front of the chest.
Treatment is adjusted for age and bone health. In an older person with osteoporosis or a fragile spine, gentle mobilisation replaces anything forceful, and any technique that stresses the neck is avoided if there is dizziness or a history of certain vascular problems.
Seek urgent care for neck pain after an accident or fall, with fever and a stiff neck that cannot bend forward, with severe headache, with new weakness or numbness in an arm or leg, with difficulty walking, or with dizziness, double vision or slurred speech. Hands-on treatment is not appropriate until these have been assessed.
