Manual therapy is hands-on treatment: gentle joint movements, stretches and soft-tissue techniques that ease stiffness and pain. Many homes in Narayan Peth have no treatment couch, and floor mattresses and low beds are common. The physiotherapist adapts the technique to the surface, screens for safety first and combines it with exercise.
"Manual therapy" covers the techniques a physiotherapist does with the hands: moving a stiff joint through a small range, stretching a tight muscle, pressing on a tender area, or guiding the body into a position that eases pain. It is usually part of a wider plan with exercise, not a treatment on its own, and it is chosen to match what the examination finds.
The manual therapy guide describes the main techniques. This page deals with a practical question that comes up in old-city homes: how hands-on treatment works without a treatment couch.
Techniques usually fall into a few groups. Joint mobilisation applies slow, graded movements to a stiff joint. Soft-tissue techniques work on tight muscle and fascia, and are related to the approach described under myofascial release therapy. Stretching and positioning release tight areas and improve range. Manipulation, meaning a quick, small thrust to a joint, is a separate and less commonly used skill, and is reserved for suitable cases.
The evidence is encouraging for neck and back pain, and for some joint stiffness, mainly as a short-term way to reduce pain and improve movement alongside exercise. It does not repair a damaged structure, and benefit that is not followed by exercise tends to fade. That is why we pair it with a plan. For a stiff, painful lower back, back pain physiotherapy shows the full approach.
The physiotherapist first asks about recent injury, surgery, osteoporosis, cancer history, blood-thinning medicines, numbness, unexplained weight loss and any medical condition. They examine the area for signs that should be seen by a doctor first. Strong techniques are avoided in thin bones, after a recent fracture or surgery, over inflamed or infected tissue, and where there are signs of nerve compression that is worsening. Neck techniques get particular caution, with screening of the neck's blood vessels and nerves.
You will be asked to wear loose, comfortable clothes. The physiotherapist explains what they plan to do, asks for your consent and checks in with you during. Treatment may take part of the visit, and the rest is spent on exercise and advice. Afterwards you might feel looser, a little sore, or tired, and a short walk often helps. We also note how the movement that bothered you has changed, and decide whether to repeat the technique.
Neck pain after a fall or accident, with severe headache, trouble speaking, dizziness or numbness in the face or limbs, is an emergency. So is back pain with loss of bladder or bowel control.
