Subjective

For two years, the first ten minutes of nearly every new patient visit went the same way. I'd explain the assessment, the plan, the fee — and almost without fail: "Bhaiya thoda kam kar do."

Sometimes ₹500 became ₹400. Sometimes a "package discount" got invented on the spot. It was never really about the ₹100. It was about where the visit sat in the patient's head — somewhere between a delivery guy and a domestic help. A person who turns up at the door with a bag, not a clinic that sends a clinician.

On paper, nothing was actually wrong. Fully booked. 8–9 home visits a day. Decent outcomes, decent word-of-mouth. But income had been flat for a long time, and every new patient meant negotiating the price from zero, again.

Assessment

This was never a pricing problem. It was a presentation problem. Show up as "a physio who visits homes," and you're a freelancer — and freelancers get negotiated with, every time, by everyone, because that's the category they've been filed under.

Show up as a practice — branded invoice, branded WhatsApp, a name attached to the visit before treatment even starts — and the patient is paying for a system, not haggling with a guy. Same hands. Different category.

Plan — treatment protocol

What actually changed, in order:

  1. Name the practice. Give it a name that isn't your name — "Proper Care Physiotherapy," not "Tushar Physio." This one line does more work than people expect.
  2. Replace the handwritten chit. Every patient gets a proper invoice with the clinic name and logo on it — not a page torn from a notebook.
  3. Move communication off your personal number. Appointment confirmations and reminders go out from "[Clinic Name]" on WhatsApp, not from your phone.
  4. Give patients a portal, not a chat thread. Treatment notes, progress, next appointment — somewhere that isn't a WhatsApp message that gets buried by tomorrow.
  5. Certificates on letterhead, same day. If a patient needs something for office or insurance, it's ready that day — not "I'll write something, give me a day."
